The Highland Block Nurse Program enhances the quality of life for seniors and enables them to remain in their own homes. The professional in-home nursing care and volunteer support services help delay or avoid moving to an assisted living or nursing home facility. With a call to the Highland Block Nurse Program, seniors and their families receive help in coordinating the services they need while they continue to live at home.

In-home Nursing Services:

  • Health care assessments, ongoing case management and referrals to appropriate community resources.
  • Medication monitoring, set-ups and pre-filling syringes.
  • Blood pressure, weight checks and heart and lung assessments.
  • Monitoring of progress after a hospital stay.
  • Licensed home health aides provide assistance with personal care (baths, shampoos, medically prescribed exercises), some light housekeeping and meal preparation
  • In-home foot care.

Volunteer Services:

  • Friendly visits by trained volunteers
  • Companion for outings
  • Transportation for appointments
  • Shopping assistance
  • Assistance with errands
  • Telephone Reassurance

Referral Services:

  • Health promotion/wellness materials
  • Housekeeping/chore service
  • Home delivered meals
  • Grocery and medication delivery
  • Health care equipment and supplies
  • Transportation
  • Adult day care
  • Respite care

The Highland Block Nurse Program accepts Medicare, Medicaid, and private insurance and has a sliding fee scale based on monthly income for nursing and home health aide services.

The program's Class "A" Home Care License (licensed under Franciscan Home Care) provides private pay care in and around the designated service area. Due to funding sources, the sliding fee scale does not apply beyond the primary service area.

Highland Block Nurse clients receive priority admission to St. Mary's Home should they require nursing home care. This is based on room availability at St. Mary's Home.
