Franciscan Home Health serves primarily those who are acutely ill and need short-term care. Our agency is certified by Medicare. Often, services are needed following a hospital stay or illness. To determine if our St. Paul home health care program meets your needs, a Registered Nurse will visit you at home for an assessment. The nurse will then work with you and your doctor to set up individualized services or make referrals to more appropriate services within our Franciscan Health Community.
Because your needs are unique, Franciscan Health Community will develop an individual nursing care plan in consultation with your doctor. We make adjustments as necessary to meet any changes in your condition.
Our St. Paul home health care staff includes Registered Nurses, Home Health Aides, Physical, Occupational and Speech Therapists and Social Workers and access to a Registered Dietitian.
