Transitional Care
When you need short-term rehabilitation and care, the Transitional Care Unit in St. Paul offers the right mix of care and services. The TCU serves clients age 18 years and older who have experienced a major medical event. The staff’s goal is to help rehabilitate them to their highest functional status so they can return to the living environment of their choice.
The TCU offers comfortable private rooms with televisions, DVD players, clock radios, telephones and electric hi-lo beds with pressure-relieving mattresses. Clients may utilize the library and borrow books and movies to enjoy during their stays.
Physical, occupational, and speech therapies are offered seven days per week within the Transitional Care Unit.
Once a client leaves the TCU they can choose to take advantage of other FHC services, such as Home Health Care or home-delivered meals.
