Home Health Care

FHC offers two high quality, home health care programs in St. Paul – Highland Block Nurse Program and Franciscan Home Health – to meet the diverse needs of seniors and their families.

Highland Block Nurse Program utilizes professional staff and volunteers to provide nursing services; and personal care such as bathing assistance, companionship, and assistance with errands and transportation. Home health care eases some of the burdens of daily living so seniors can remain in their homes for as long as possible.

Staff also serve as a respite for caregivers who may become overwhelmed by providing constant care for a loved one. Home health care often results in improved status for the senior: physical and cognitive well being often improve if elders eat better, are well hydrated, and take their medications on time. They may face fewer fall risks and have fewer emergency room visits.

The Highland Block Nurse Program relies upon a dedicated team of volunteers to befriend and assist seniors. Volunteers drive clients to doctor appointments or the grocery store, or just spend time visiting or baking cookies together.

The Highland Block Nurse Program, which is supported by donor gifts and grants, also accepts Medicare, Medicaid, private insurance, and has a sliding fee scale based on monthly income.

Franciscan Home Health serves adults who are acutely ill, homebound and require skilled care. The program is state-licensed and Medicare/Medicaid-certified serving clients in Ramsey County, and portions of Hennepin and Dakota Counties.

The care team members provide quality care with integrity, compassion, trust, and respect. The Franciscan Home Health staff includes registered nurses, home health aides, social workers, dietary staff, and physical/occupational/speech therapists.

Skilled care services include health assessments, skilled nursing care, medication management, wound care, physical therapy, and speech therapy.

The program’s home health aides assist with bathing and dressing, meal preparation, medication reminders, assistance with walking, and light housekeeping duties.
