Admission
The criteria for admission are a diagnosis of incurable cancer with all aggressive care completed, and the inability to pay for the needed skilled care. Patients are welcome regardless of faith, age, race, gender, national origin, handicap or marital status.
The following information is required prior to admission:
- Application
- Recent history and physical
- Chest x-ray taken within the last 3 months
- A pathology report
- An advanced directive (Do not resuscitate/Do not intubate status)
- Most recent medication list
If possible, we encourage that the patient’s family or legal guardian, and the patient if possible, visit the Our Lady of Good Counsel prior to admission. As we often have a waiting list, it is advisable to submit an application as soon as the need for placement is determined.
