St. Mary's Home is a 100-bed Catholic St. Paul nursing home offering a range of skilled nursing services to the Twin Cities area, including transitional care and hospice care.
Our goal at St. Mary's Home is to keep residents as independent as possible with the highest quality of life. Our St. Paul nursing home accomplishes this through attentive nursing care and knowledgeable staff who focus on keeping residents as comfortable as possible. The staff at St. Mary's Home work closely as a team with residents' physicians to provide excellent, high quality care.
Our nursing home facility offers all private rooms, dining rooms on each floor, a beautiful large chapel, lounge areas, sunrooms, and a fenced patio for outdoor activities. It is a smoke-free facility.
St. Mary's Home is Medicare and Medicaid certified. Rates are determined by the level of care required and by the type of room desired.

Transitional Care Unit
Our Transitional Care Unit serves clients age 18 years and older.
These individuals have experienced a major medical event. Our goal is to transition them to their highest functional status and return them to the environment of their choice.
We offer newly remodeled/decorated private rooms which include televisions, DVD players, clock radios, phones and electric hi-lo beds with pressure relieving mattresses. Our library lends books and movies to residents during their stays.
Physical, Occupational, and Speech Therapies are offered seven days per week. Our Therapy Department is conveniently located on our Transitional Care Unit.
By initiating discharge planning on admission, we empower residents to assist with their individual ongoing needs. Residents stay in charge as the planning process evolves. An interdisciplinary team assists residents in decisions about their care by carefully considering all available information. Residents are supplied with options concerning services they may require post-discharge and are assisted with arranging those services they choose. Franciscan Health Community offers home health care through Franciscan Home Health described elsewhere in this web site.
Application Process
Those interested in admission to St. Mary's Home may contact the admissions coordinator for information on the application process. There are no application fees. Persons using other Franciscan Health Community services receive priority admission to the nursing home based on room availability.
